Under normal conditions in the rat brain, tyrosine hydroxylase, the rate-limiting enzyme on the pathway from tyrosine to norepinephrine, is about 75-percent saturated with tyrosine.5 There has been much less research on the use of norepinephrine precursors for treating depression than the use of serotonin precursors. Two 1980 case studies suggested Ltyrosine may have potential as an antidepressant, but both had a very small sample size.27, 28 Later in the 1980s, a four-week clinical trial treated 65 outpatients with major depression in a double-blind comparison of L-tyrosine 100 mg kg day ; , imipramine, or placebo. It found no evidence that L-tyrosine had antidepressant activity.29 In a double-blind study reported in 1979, DL-phenylalanine 150-200 mg day ; or imipramine was administered to 40 depressed patients 20 in each group ; for one month. No statistical difference was found between the two groups using the Hamilton Depression Scale and a self-rating questionnaire, 30 leading to a conclusion that DL-phenylalanine DLPA ; might have antidepressant properties. Eumycetoma due to Trichophyton tonsurans Manz B, Nenoff P, Mittag M, Rytter M, Haustein UF Dept. of Dermatology, University Leipzig, Germany A 16 year old asylem from Senegal, who had worked there in agriculture, suffered from several injuries at his feet and shanks by thorns. Since 8 months there is a slow progredient tumour at his left leg with pain on walking. Multiple biopsies of the skin furnished the histological evidence of granulomatous inflammation; causative agents actinomycetes, mould fungus ; could not be detected. The suspicion diagnosis ensued mycetoma. The left foot impressed with a tumourous nodular swelling, partly with fistulas, partly with ulceration. There were no grains to express. Therapy was initiated locally with povidon jod Betaisodona solution ; , systemically with trimethoprim sulfamethoxazole Bactrim forte ; 2 x 1, itraconazole Sempera ; 2 x 200 mg as well as rifampicin 2 x 300 mg. A definitive diagnosis was achieved only after a deep biopsy which was performed in general anaesthesia: beside granulomatous tissue and inflammatory infiltration with foreign body giant cells, little grains with bulky septated hyphae were recognized in the deep layers of the corium respectively subcutis. A dermatophyte of the genus Trichophyton grew from the tissue at 26C and 37C on Sabouraud`s 4%-dextrose medium, with and without Actidione ; . Pediatric bactrim dosing

In a dose of 60 mg kg per day Proloprim; Burroughs Wellcome Co., Research Triangle Park, N.C. ; was added to the daily food supply. TMP at 50 mg kg per day and SMZ at 250 mg kg per day were given in the drinking water Bactrim suspension; Hoffmann-La Roche Inc., Nutley, N.J. ; . The drug preparations were made freshly every other day for oral use. DFD was added to pulverized food pellets, following which a paste of the mixture was made, pressed into pellets, and allowed to dry. The weight of the food and volume of drinking water consumed daily were measured. The medicated water and food were available continuously until consumed. The amount of rations consumed during the previous 2 days was used to calculate the amount to prepare for the next 2 days. The usual food and water wastage was not measured but was estimated to be about 15% of the total. All of the medications were taken without difficulty. Groups of 10 rats were given either 0.5, 5.0, 25.0, or 125.0 mg of DFD per kg per day during a 6-week period of immunosuppression with dexamethasone. A control group was immunosuppressed but did not receive DFD. The animals were sacrificed at the end of 6 weeks of study, and the lungs were examined for P. carinii pneumonitis. The results are given in Table 1. All of the controls were infected. The lowest dose of DFD, 0.5 mg kg per day, had no discernible effect. The 5.0-mg dose prevented infection in half of the animals, establishing the effective dose for 50% of subjects. Doses of 25 and 125 mg kg per day were totally effective. Groups of 10 rats were given dexamethasone and tetracycline for S weeks, during which time P. carinii pneumonitis had become established. At this time, DFD was administered at dose levels of 0.5, 5.0, and 25.0 mg kg per day, with and without TMP, for 3 weeks. The animals were sacrificed after 8 weeks of immunosuppression and 3 weeks of treatment. One group of animals served as untreated controls, and one group received TMP-SMZ, a drug known to be effective in the treatment of P. carinii pneumonitis. The results are summarized in Table 1. DFD was clearly effective at the dose level of 25.0 mg kg per day in the treatment of animals with P. carinii pneumonitis when compared with the untreated control group. The combination of TMP with DFD was more efficient than DFD alone. The dosage of 25.0 mg kg per day in combination with TMP was as effective therapeutically as TMP-SMZ. Discussion. Previous studies showed almost identical results with dapsone at the same doses as used for DFD in this trial 6 ; . Thus, whatever advantage DFD might have over.
